Clinical fit: Patient must have a qualifying skilled need, and be under the care of a physician, NP, or PA who will sign the plan of care.

How fast will I hear back?
Within 2 business hours, usually much faster, Mon–Fri 9a–5p. If you are sending late in the day and need confirmation before you leave, call the number — a person answers.
What payment do you accept?
Private pay today; our Medicare certification is in progress and this page will be updated the day it completes. We confirm the arrangement with the family before the first visit — you do not have to pre-verify anything.
Are you taking new patients right now?
Yes — for adult and pediatric care across our service area. Capacity in some specialty pediatric areas (trach/vent, complex private duty) is set by clinician availability; we will be straight with you on the first call about timing.
What if the patient isn't a fit for home health?
We'll tell you on the first call and point you toward the right partner — outpatient, hospice, palliative, SNF, whatever fits. We'd rather not take a referral we can't serve well.
What's the right channel for PHI?
Phone or secure fax for anything with patient identifiers, diagnoses, or clinical detail. The web form is for general "I'm sending you a referral, here's the context" — fine for high-level context (no names, initials or dates of birth), not for clinical handoffs.
One call. We take it from here.
Discharge planners get the direct line, not a voicemail tree. Fax or call — we'll acknowledge fast, set the patient up, and loop you when the first visit is confirmed.
