The short version
This website is a way to learn about Vegas Home Health Partners and start a conversation. The forms here are for general inquiries. We don't sell or rent your information, and we don't share it for marketing. The service providers that run this website are listed below. Once you become a patient, your protected health information (PHI) is handled under HIPAA — but the website itself is not the channel for sensitive medical details.
What we collect
From the website: name, phone, email, zip code, type of care needed, and whatever you write in the message field on our contact or referral forms. That's it. No advertising cookies, no retargeting pixels, no identifier-based analytics.
During care: the protected health information needed to provide skilled home health services. This is governed by HIPAA, your physician's plan of care, and our Notice of Privacy Practices, which we provide at the start of care.
How we use it
To respond to your inquiry, schedule visits, coordinate with your physician, handle billing for your care, and provide care. We don't use your information for marketing to you outside the home health context, and we don't share it with marketing or analytics companies.
Who sees it
Inside Vegas Home Health Partners: only the people who need to (intake coordinator, your assigned clinicians, billing).
Outside Vegas Home Health Partners: only when required for care (your physician, the hospital that referred you) or required by law (state regulators, court orders). We have Business Associate Agreements with our HIPAA-covered vendors (email, EMR, fax).
Service providers, cookies and measurement
Hosting: this website is served by Cloudflare, which processes your IP address to deliver pages and block abuse. Forms: the contact and referral forms are delivered through Web3Forms, a form-delivery service, to our Google Workspace inbox (Google Workspace is covered by a Business Associate Agreement; Web3Forms is not, which is why we ask you not to put medical details in the forms). Fonts: fonts load from Google Fonts, which logs the requesting IP address but does not set cookies.
Measurement: we use Cloudflare Web Analytics, a cookieless, aggregate measurement tool that does not store IP addresses and cannot identify you. We don't use Google Analytics, advertising pixels, session recording, or any identifier-based tracking. We don't sell browsing data because we don't collect it.
Security
Our care delivery (clinical visits, EMR, billing) is HIPAA-compliant with business associate agreements in place where required. The website forms route through a third-party form service and standard email, which is appropriate for general inquiries but is not the right channel for protected health information — please use the phone or fax for that.
Text messages
If you text us or ask us to text you, message and data rates may apply. Reply STOP at any time to opt out. We do not send marketing texts.
Your rights
You can ask us what we have on file about you, ask us to correct something, ask us to delete it (subject to medical record retention rules), or ask us to stop contacting you. Email admin@vegashomehealthpartners.com or call (725) 322-6968.
